Sometimes your attic insulation is blocking the soffit vents, sometimes there is not enough soffit or ridge vent area (especially if you have a hip roof). Sometimes there is not enough insulation, or gaps of insulation in the attic.
Most ice dams form near the lower edges of the roof. This is why today’s Massachusetts construction code requires the use of ice and water shield 3 feet in side a heated wall. Most roofing companies today will actually go 6 feet, which definitely helps with New England winters. With ice and water shield, when the water backs up under the shingles, it runs down the underlayment (ice and water shield), to the roof edges instead of into your home.
